Welcome, Caseworkers 

Home Start Hope partners with select social service agencies to support women transitioning from homelessness to independent living. This page outlines our eligibility criteria, referral process, and requirements for working with our program.

STEP 1: GET STARTED

Caseworkers at Current Partner Agencies

If your agency already partners with Home Start Hope AND you are a new caseworker (or new to the program), please complete the Caseworker Intake Form to schedule your tour and training.

If your organization is not currently a referring partner, please complete the Partner Agency Application to request partnership with Home Start Hope.

PARTNER AGENCY APPLICATION

STEP 2: REQUIRED TOUR AND TRAINING

All caseworkers must complete a Home Start Hope tour and training before submitting client referrals. This ensures familiarity with our program, what clients receive, and the correct referral and scheduling steps. The tour and training takes approximately 30-45 minutes to complete. The tour and training takes place at Home Start Hope: 90 Empire Blvd. Rochester 14609.

Important note: Home Start Hope is not staffed on-site. Training is scheduled intentionally, and attendance is important. If you are unable to attend a scheduled tour and training, please email us in advance so time can be used appropriately.

CASEWORKER TRAINING CALENDAR

STEP 3: WELCOME EMAIL

After completing the training, you’ll receive a welcome email within a few days that includes your passcode and other important program details. Please keep this email for future reference.

STEP 4: SUBMIT AND SCHEDULE

If your client meets the Eligibility Criteria, use the link below to submit a referral and schedule an appointment. Instructions are on the calendar, in the monthly reminder email, and in the Resources section below.

Client Eligibility Criteria

Home Start Hope serves individuals who meet all of the following criteria:

  • Are transitioning from homelessness to independent living
  • Are women who were assigned female at birth
  • Are single heads of household
  • Have no spouse, partner, or significant other residing in or financially contributing to the household
  • Are the sole adult responsible for household decision-making and expenses

Referrals that fall outside these criteria are not eligible for services.
